Shedding Light on the Dark Spaces in Healthcare

Posted by Travis Bond

Mar 8, 2017 4:39:28 PM

Between office visits, between different providers, or during transitions in care, patients are often navigating a fragmented healthcare system on their own. In turn, physicians miss out on vital information, having to rely on patients to tell them if they saw a different doctor, experienced an emergency, or started taking a new medication. These voids in communication, care, and guidance are often called the dark spaces in healthcare.

Care coordination services are designed to shed light on these dark spaces.

How mHealth Can Enhance a Physician’s Brand

In an increasingly digital world, patients are becoming more tech- and mobile-savvy in the way they search for and experience healthcare. But far too many physicians are not taking advantage of the mobile revolution, or using medical apps or other mobile health technology to enhance their care delivery outside the point of care.

It’s a disconnect that needs to change as healthcare providers who don’t engage with mobile technology risk being compared to taxicabs in the world of Uber. Further, as the industry continues the volume-to-value revolution, patients—especially those with chronic or complex conditions—will require communication outside the practice walls. Being seen as outdated and inaccessible in an online environment could affect a consumer’s perception of a physician’s ability to provide the highest-quality care.

Physicians can do a lot in the exam room. They can ask questions, listen attentively, take notes, render a diagnosis, and prescribe treatment and medications. What a physician cannot do, however, is significantly improve patient outcomes by only making health recommendations just during the visit, or by only sending lab results over a secure patient portal.

In order to truly impact care, physicians must extend their reach and presence beyond the practice walls, to the places where patients live, work and play.

Chronic Care Management: Steps in the Right Direction

Before Chronic Care Management (CCM) was a code, we felt the healthcare system was lacking in care coordination and that someone needed to shed light on the dark spaces (the voids between office visits and transitions in the healthcare system) and help patients make sense of fragmented care. We created CareSync in 2011 to put patients at the center of their care and were already performing chronic care management when CMS announced the code and began reimbursing for it in 2015.
