If you’re one of the nearly 2,900 primary care physicians who began participating in Round 1 of Comprehensive Primary Care Plus (CPC+), or you’re one of the thousands expected to apply for Round 2, you know it’s not without its challenges. This five-year program involves robust care delivery requirements, expectations to meet certain quality and utilization measures, and risk associated with receiving prospective payments. But it also presents an exciting opportunity to be a key player in changing the way practices deliver care, payers pay for it, and patients experience it.

New CCM Opportunities for RHCs and FQHCs

After waiting a year to be able to participate in Medicare’s Chronic Care Management (CCM) program, Rural Health Clinics (RHCs) and Federally Qualified Health Centers (FQHCs) still faced a formidable obstacle to implementing CCM.

In 2016, they could be reimbursed for providing CCM to their patients, but they were unable to contract with third parties to help them do so. Because CCM had to be provided by their clinical staff under direct supervision by the practitioner, offering CCM became challenging at best.

How mHealth Can Enhance a Physician’s Brand

In an increasingly digital world, patients are becoming more tech- and mobile-savvy in the way they search for and experience healthcare. But far too many physicians are not taking advantage of the mobile revolution, or using medical apps or other mobile health technology to enhance their care delivery outside the point of care.

It’s a disconnect that needs to change as healthcare providers who don’t engage with mobile technology risk being compared to taxicabs in the world of Uber. Further, as the industry continues the volume-to-value revolution, patients—especially those with chronic or complex conditions—will require communication outside the practice walls. Being seen as outdated and inaccessible in an online environment could affect a consumer’s perception of a physician’s ability to provide the highest-quality care.

Things to Consider Before Starting a CCM Program

Posted by Mark Nalywajko

One-on-One with Amy Gleason: Part 2

There is a lot to seriously consider before you jump right in to administering your own Chronic Care Management (CCM) program. Compliance should be at the very top of your must-have list. After CMS gets it off the ground, they will start enforcing its requirements like nobody’s business, then dish out the fines for those who are not compliant. Many providers will also need to decide if they have the manpower to administer the program themselves, or if they want to partner with a CCM provider.

Beware the bargain hunters that are looking for extreme deals, in order to reduce costs. Corners will be cut, and cheap programs risk compliance with CMS resulting in potential fines. Finally, providers need to find the best way to integrate CCM with their practice EMR. Finding a custom solution instead of a one size fits all bargain basement product, is certainly a challenge. In Part 2 of a special series, RN, Co-Founder and Chief Operating Officer of CareSync, Amy Gleason, answers questions that will help providers overcome these challenges.
