CMS Requirements for Billing for Chronic Care Management

We spend a lot of time working with medical organizations throughout the US who are excited at the potential outcomes of implementing a Chronic Care Management (CCM) program for their organization, but overwhelmed by the requirements put forth by CMS.

Today, we’ve pulled together the specific requirements that you’ll need to accomplish to properly bill CPT 99490.

The Comprehensive Care Plan

Chronic Care Management is all about putting the patient at the center of their care. In order to accomplish this (and get reimbursed), providers are required to build a comprehensive, patient-centered health summary and Care Plan for each eligible patient.

This care plan needs to include all of the current medical records from all of the patient’s providers.
