Awhile back, I wrote about my interesting first experience as an engaged patient, stuck in the emergency room during a conference I was excited to be at and briefly mentioned the appointment I had a week later with my primary care provider.

For those of you who missed it, I got sick when I was away from home back in June, but was able to avoid a not-so-fun procedure (in the middle of the night, no less) because I had the results from the exact same (also not fun) procedure I'd had done about four weeks prior at a regular check up.  On my iPhone.

A week after my ER visit, I followed up with my provider, ER records in hand, and we went through all of the pieces of data that we knew to be true.  Drugs that had been taken and what they were taken for, procedures that ruled out certain diagnoses, and other factors related to my complaints.  Whlie we didn't have all of the answers, we at least had all of the answers that we knew in front of us, organized in a way that we could make the best decisions for my health.  careplan quote

Instead of leaving with yet another prescription for yet another antibiotic, I left with a care plan.  My provider sat down next to me, and we made a realistic, measurable plan for me to follow for the next 6 weeks.  He told me that if 5% of his patients were as organized as I am, his job would be 95% easier.

Well, fast forward 6 weeks, and it's time to go back for my follow up appointment to see just how well that care plan is working.  Instead of my normal scrambling around for various notes, lists, and questions I want to ask, I've been documenting everything within my CareSync account all along.  I've created notes that include what I am looking to accomplish during the visit, I've added lab results and have tracked my symptoms.  I will be faxing the summary report to my provider as soon as I am finished writing this blog post, and it will be waiting for him on the fax machine when he gets to the office tomorrow morning.

I feel prepared for my appointment, and hope that this is the last one for awhile!  That? Is to be continued.
